COVID-19 Telehealth/In-person Treatment

 

Barring a change in public health orders, I am seeing clients in-person in my office at 2400 Fenton Street, Suite 205, Chula Vista, CA, 91914. Please read the entire message below for essential information about this.

In-person sessions are optional. Before deciding if you want to attend in-person sessions, please go to your client portal (new clients will need to create a client portal), click on My Docs & Forms, and review the Covid-19 In-person Treatment Consent. After reading this form, if you decide you wish to do your sessions in person, sign and submit this form. If you do not sign and submit this form prior to your session, you will not be seen in-person.

If you would like to attend your sessions in-person: Please make sure you select an "In-person" session when you are scheduling your appointments online.

Jeff has been fully vaccinated and boosted, and will not be wearing a mask during sessions unless otherwise posted at the office.

If you prefer to have your sessions via telehealth: you may do so at your discretion as long as your insurance covers it. You must select a  “Telehealth ” appointment every time you schedule. This is the only way I will know that you intend to have your session via telehealth. Please note, although this is not common, you are responsible for any fees you may incur if your insurance stops covering telehealth without notice.
